SINGAPORE, 24 Jul 2024 – Accredify, Asia-Pacific’s leading verifiable credentials platform provider and the National Healthcare Group (NHG) have announced a strategic partnership to develop the NHG Cares Partners Portal. This collaboration aims to enhance the integration of community and primary care resources, aligning with NHG’s vision of Adding Years of Healthy Life with a Community of Care (CoC) in every neighbourhood across Central and North Singapore.

The Portal is set to revolutionalise community care by:

• Simplifying programme management for community partners: The Portal automatically syncs programmes with Health Kampung, NHG’s health and social care marketplace. It streamlines registration and attendance tracking, allowing community partners to monitor programme demand and adjust capacity accordingly. It also provides real-time updates on programme changes for seamless interactions with residents, facilitating easy and timely access to community care.

• Leveraging aggregated PopHealth data: Insights from anonymised PopHealth data empower community partners with a comprehensive understanding of residents’ profiles. This enables them to design and deliver tailored community-based programmes that meet residents’ needs and preferences, enhancing their overall well-being and quality of life.

• Empowering residents to take ownership of their health journey: The Portal allows community partners to issue e-certificates to residents when they accomplish a health or fitness goal. This recognition not only celebrates residents’ achievements but also serves to motivate residents to continually strive for healthier lifestyle.

Since its launch in April 2024, the Portal has onboarded more than 20 community partners and automated more than 400 programmes across some 100 sites in Central and North Singapore.

Looking forward, the NHG Cares Partners Portal will also set the foundation for improved care continuity. Plans are underway to provide shared care services for NHG’s assigned residents through referrals from GPs and/or community partners through the Portal, such as screening services to residents by GPs, social prescribing through community programmes aggregated from partners, and more.
